Recuperative Care Case Manager II

JOB SUMMARY:

This role will work with the Recuperative Care team supporting older adults in their transition from hospital discharge to stable housing. Will include assistance with benefit and financial applications, permanent housing placement and implementation of the care plan. Will assist with coordination of services with Curry and other service partners.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

●      Conduct screenings and assessments, provide comprehensive case management services, including collaborative treatment planning and service linkage.

●      Ensure timely and accurate documentation of client interactions, progress, and outcomes in accordance with established protocols.

●      Ongoing coordination of recuperative services and resource development.

●      Maintain electronic medical and service records.

●      Follow all agency protocols, participate in all required agency meetings/training, and clinical supervision.

●      Manage and maintain an assigned caseload.

●      Perform other duties as assigned.

 

JOB REQUIREMENTS:

●      Master’s degree in social work required.

●      2-3 years of experience working with older adults is preferred.

●      Proof of negative or inactive TB required. Curry Senior Center will provide or reimburse for the cost of a TB test after an offer of employment has been accepted by both parties. Proof of a negative TB test within 90 days will also be accepted.

●      Must be a self-starter with excellent follow-through skills.

●      Willingness to work in a community-based setting in the Tenderloin neighborhood with low-income, frail and culturally diverse older adults who may have multiple behavioral and physical health problems.

●      Ability to work with a team of diverse and multi-disciplinary professionals.

●      Ability to maintain a professional demeanor with good interpersonal and communication skills.

●      Effective oral and written communications skills with the ability to express ideas in a clear, thorough, concise manner.

●      Strong interpersonal, communication and organizational skills. Demonstrated ability to build rapport and maintain supportive client relationships.

●      High degree of skill in grammar, spelling and punctuation.

●      Ability to maintain the confidentiality of all information obtained.

●      Strong organizational skills and the ability to manage multiple priorities effectively.

●      High level of analytical problem-solving skills.

●      Excellent time management skills, including taking initiative in professional development.

●      Must be action-oriented and have the ability to work independently and as a team player.

●      Strong computer skills such as Chrome, Google suite, Apple iOS, Office suite.

 

PHYSICAL REQUIREMENTS/DEMANDS:

The incumbent is in a non-confined office-type setting in which they are free to move about at will. The Environment is mostly clean and comfortable but may include some minor annoyances such as noise, odors, drafts, etc. Work will be performed utilizing a computer, sitting at a desk/chair or standing while speaking and interacting with clients. Position will require average finger dexterity to operate normal office equipment and systems.

Bending:        15 % of the workday

Twisting:         15 % of the workday

Lifting:            15 % of the workday/Moderate, generally not over 10 pounds
