Responsibilities:
- The Intake Coordinator will be responsible for managing the intake process for patients, including scheduling appointments, obtaining medical records, and coordinating with other departments.
- Answer incoming calls and provide information about available services
- Triage incoming calls related to patients, families, physicians, and staff Initiate referral process as outlined in department specific procedure
- Collect and enter patient information into the system accurately and efficiently
- Verify insurance coverage and obtain necessary authorizations
- Input new patients, new vendors, and new field staff into software system
- Provide patient education on hospice and palliative care services
- Schedule appointments and coordinate with medical staff - Maintain confidentiality of patient information and adhere to HIPAA regulations
- Assist with administrative tasks as needed
- Check the monthly schedule weekly and update on the go changes.
Qualifications:
- LVN LICENCE preferred
- BILINGUAL preferred - Previous experience working in a Hospice Agency.
- Excellent communication skills, both verbal and written
- Strong attention to detail and ability to multitask effectively
- Proficient in computer skills, including data entry and electronic medical records systems
- Ability to work well under pressure and remain calm in stressful situations
- Knowledge of medical terminology is a plus
