Position Overview
Reporting to the Service Line Manager of the ESD, the Care Coordinator serves as an important connection between CMH Emergency Services, and the coordination of patient care with other CMH services, such as Primary, Specialty, and Ancillary services. The position will provide care and case management services to individual patients and their families, and directly manage the scheduling of appointments with other CMH services to ensure successful coordination of patient care.

$26.13 - $40.53 per hour, depending on experience

Education/Experience
Minimum Required:
  • Associate's degree in health or communications related field of study.
Preferred:
  • 3 years or equivalent experience in community outreach, health education, population health or related field.
  • In-depth understanding of the local health care system and community health needs, and a passion for the advocacy of health and wellness.
