Key Responsibilities:

• Coordinate home care services for patients, ensuring timely scheduling and staffing

• Maintain communication with caregivers, clients, and families

• Handle onboarding and compliance documentation for new caregivers

• Track service authorizations and update records accordingly

• Work closely with the nursing and administrative teams to ensure smooth care delivery

Qualifications:

• Previous experience in home care or healthcare coordination preferred

• Strong organizational and communication skills
