We are seeking a compassionate and experienced professional to join our team on a part-time basis. In this role, you will support patients and families within the hospital setting as they explore and transition to hospice care. Responsibilities include providing counseling, facilitating informed decision-making, and coordinating safe and appropriate discharge plans.
This position collaborates closely with hospital staff, hospice and home care interdisciplinary teams, admissions and finance departments, and community resources to ensure seamless care and communication. The role also provides direct social work services to hospice GIP patients and their families, helping to ensure comfort, clarity, and continuity throughout the hospice admission process.
Education and Experience:
Current LMSW or LCSW license in New York State
Minimum of two years’ experience in healthcare, preferably in hospice, hospital, or home care settings
Strong ability to represent the hospice agency professionally and interact tactfully with patients, families, and community partners
Familiarity with hospice eligibility criteria, regulatory requirements, Joint Commission standards, and private pay practices preferred
Excellent communication, negotiation, and public relations skills
Demonstrated ability to work independently while maintaining strong organizational skills, flexibility, and collaboration
Commitment to and understanding of the hospice philosophy
