Maintain accurate documentation and case records in compliance with institutional policies.
Advocate for patient rights and access by collaborating with insurance providers, legal aid, and community organizations.
Build and manage resource directories and referral networks to connect patients with external support programs and social services.

Qualifications:

Master’s degree in Social Work, Clinical Psychology, Psychology, or a related field. Concentrations in Gerontology, Pediatrics, Public Health, Substance Abuse, or Mental Health are preferred.
Minimum of 5 years of clinical experience in a relevant field.
An active, unrestricted Licensed Clinical Social Worker (LCSW) license may substitute for 2 years of required experience.
