Job Title: Social WorkerLocation: Remote (based out of Santa Barbara)Experience Level: IntermediateOverview:We’re looking for a compassionate and resourceful Social Worker to support patients and families navigating the healthcare system. This role focuses on advocacy, coordination of services, and ensuring continuity of care through strong communication and case management.Key Responsibilities:
  • Coordinate support services such as transportation, housing assistance, and financial aid for patients and families.
  • Facilitate clear communication between healthcare providers, patients, and families to ensure understanding of care plans and available resources.
  • Maintain accurate documentation and case records in compliance with institutional policies.
  • Advocate for patient rights and access by collaborating with insurance providers, legal aid, and community organizations.
  • Build and manage resource directories and referral networks to connect patients with external support programs and social services.

Qualifications:
  • Master’s degree in Social Work, Clinical Psychology, Psychology, or a related field. Concentrations in Gerontology, Pediatrics, Public Health, Substance Abuse, or Mental Health are preferred.
  • Minimum of 5 years of clinical experience in a relevant field.
  • An active, unrestricted Licensed Clinical Social Worker (LCSW) license may substitute for 2 years of required experience.
