Under the supervision of the hospital Chief of Social Work Service, and in consultation with the statewide DSH Program Director for Discharge and Community Integration (DCI), the Discharge and Community Integration Specialist is a coordinator of hospital-based discharge efforts for civil and forensic patients within DSH, including but not limited to: training, resource development, strategic partnership, program implementation & evaluation, ongoing data collection and monitoring, and policies and procedures. Collaborates with the Forensic Conditional Release Program (CONREP) as it relates to discharge and community integration of relevant legal commitments and implementation of the new referral process. Serves as a trainer for staff, and as hospital liaison to the statewide Discharge and Community Integration program. Occasional, limited travel may occur and reimbursement will be in accordance with BU agreements and standard travel policies.

Working Conditions
Occasional, limited travel may occur and reimbursement will be in accordance with BU agreements and standard travel policies.
- Hybrid telework is available based on operational needs and is subject to change, with a current minimum three in-office work days per week requirement. In office work requirements for new employees may increase during the first thirty (30) days of employment based on training needs.
The work hours for this position will be set sometime between 8:00 am and 5:00 pm Monday through Friday.

Desirable Qualifications
- An experienced social worker with the ability to plan and execute complex organizational needs assessments; conduct comprehensive evidenced- based literature reviews; complete collaborative action plans and projected budgets for leadership review
- Knowledge and understanding of the Conditional Release Program (CONREP) as well as other mental health community treatment programs
- Knowledgeable of the systems of care related to older adults, mental health, substance abuse, developmental delay, primary care, palliative care, chronic illnesses, dialysis, conservatorship, and probation and parole
- Knowledgeable of housing resources, their requirements for eligibility, and other community services such as In-Home Supportive Services(IHSS), Multipurpose Senior Services Program (MSSP), Institute on Aging (IOA), and shelter Navigator and Project Home Key
- Knowledgeable about local, state and federal benefit and entitlement programs, such as Social Security, SNAP, Medi-Cal, and General Assistance
- Experienced working with a diverse population (race, ethnicity, age, ability, language, nationality, socioeconomic status, gender, religion, and sexual orientation)
- Able to work in a politically-charged environment under high degrees of pressure and meet all deadlines
- Able to develop effective and positive working relationships with community partners such as Aging and Adult Services, Behavioral Health, and Social Security Administration
- Able to communicate clearly and concisely; ability to maintain confidentiality
- Knowledge of philosophy and function of social work systems
- Appropriate utilization of a variety of social resources
