LVN Care Coordinator – Full Time (Santa Ana Office)

We are a growing and compassionate home health company seeking a Full-Time LVN Care Coordinator to join our team in Santa Ana, CA. The ideal candidate is organized, detail-oriented, and passionate about providing excellent patient care coordination in a fast-paced environment.

Responsibilities

• Answer and route phone calls for general questions and clinical inquiries.
• Request, track, and follow up on authorizations from payors.
• Coordinate care between patients, clinicians, and referral sources.
• Obtain and transcribe faxed or telephone orders.
• Assist clinicians with opening notes and documentation.
• Process incoming and outgoing mail.
• Assist with Medicare reporting and compliance requirements.
• Refer patients to specialty clinics such as INR and Wound Care.
• Upload Service Agreements, NOMNC, and related documents into the EMR system.
• Request, organize, and upload medical records.
• Create and send fax requisitions and lab results to MDs or pharmacies.
• Order Medicare supplies, DME, and other items covered by patient insurance.
• Perform other administrative and care coordination tasks as assigned.

Qualifications

• Active LVN license (California).
• 2+ years of experience with authorization requests or care coordination preferred.
• Strong understanding of insurance plans — including IPA, HMO, and PPO.
• Skilled in insurance eligibility verification and working with various payor systems.
• Ability to multitask efficiently between phone calls and daily job responsibilities.
• Proficient in Microsoft Word and Excel.
• Spanish speaking a plus.
• Excellent communication, organization, and follow-through skills.
