Responsibilities

• Collaborate with medical staff to integrate social work services into patient care plans, ensuring holistic treatment approaches.
• Manage cases effectively by coordinating services and resources for patients, including referrals to community resources.
• Implement crisis management strategies for patients experiencing acute emotional or psychological distress.
• Offer support in senior care settings, addressing the unique challenges faced by elderly patients.

• Qualifications
• Master’s degree in Social Work (MSW) from an accredited institution is required.
• Proven experience in case management within a healthcare setting.
• Excellent crisis management skills with the ability to remain calm under pressure.
• Experience in senior care or geriatric social work is a plus.
• Exceptional interpersonal skills with the ability to build rapport with diverse patient populations.
