Now Hiring: Care Manager – Children’s Health Home Program

KidsPeace is seeking a compassionate and organized Care Manager to provide outreach, enrollment, and case coordination services to children eligible for New York’s Children's Health Home program. This full-time position plays a vital role in creating and managing personalized health care plans, coordinating with families and service providers, and supporting transitions between levels of care.

Responsibilities:

• Conduct outreach and assess eligibility for Children’s Health Home enrollment
• Document assessments and service needs
• Develop comprehensive Plans of Health Care with families and providers
• Coordinate ongoing involvement and support from relevant parties
• Track progress, maintain documentation, and facilitate transitions between health homes or levels of care

Qualifications:

• High School Diploma plus 5 years of relevant experience
• Preferred: BA degree with 2–3 years’ experience, or MA degree with 1 year
• Valid driver’s license required
