Overview
We are seeking a compassionate and dedicated Medical Social Worker to join our healthcare team. The ideal candidate will play a crucial role in providing support and resources to patients and their families, ensuring they navigate the complexities of medical care effectively. This position requires a strong background in social work, mental health counseling, and patient advocacy, with a focus on enhancing the quality of life for individuals facing health challenges.

Responsibilities

  • Conduct comprehensive patient assessments to identify needs and develop appropriate care plans.
  • Provide individual and group counseling to patients dealing with mental health issues, chronic illness, or end-of-life concerns.
  • Collaborate with healthcare providers to ensure coordinated patient care and effective treatment plans.
  • Facilitate crisis intervention and management for patients experiencing acute distress or emergencies.
  • Maintain accurate medical documentation and records in compliance with HIPAA regulations.
  • Engage in case management activities, including referrals to community resources and support services.
  • Participate in clinical research initiatives to improve patient outcomes and advance social work practices.
  • Educate patients and families about available resources, treatment options, and coping strategies.
  • Advocate for patients' rights and needs within the healthcare system.
  • Provide support in hospice care settings, focusing on comfort and quality of life for terminally ill patients.

Skills

  • Proficient in child & family counseling, clinical research, and hospital experience.
  • Strong background in data collection, hospice care, addiction counseling, behavioral therapy, and applied behavior analysis.
  • Expertise in patient care, patient assessment, medical documentation, medical terminology, and medical records management.
  • Experience with intake processes, clinical counseling, group therapy, social work practices, behavioral health interventions, and mental health counseling techniques.
  • Skilled in diagnostic evaluation, case management strategies, individual/group counseling approaches, psychotherapy methods, and motivational interviewing techniques.
  • Familiarity with assisted living environments and experience working with children is preferred.
  • Competence in crisis intervention strategies and home & community care experience is essential.
  • Knowledge of EMR systems is beneficial along with inpatient care experience or nursing home experience.
  • Proficient in cognitive behavioral therapy techniques as well as working with individuals with developmental disabilities or Alzheimer’s care needs.
  • Experience providing PTSD care or post-acute care services is valuable.
  • Understanding of CPT coding as well as ICD coding (ICD-9 & ICD-10) is advantageous for documentation purposes.
  • Background in geriatrics or pediatrics will be considered an asset.

Pay: $50.00 - $135.00 per hour
