Job Summary:
The Health Home Plus Care Manager is responsible for providing comprehensive Care Management services to specific populations including those in recovery from Physical, Mental Health and Social setbacks. The Care Manager fosters health promotion, transitional care, patient support and referral to community and social support services. The Care Manager is responsible for creating and executing a Care plan that links individuals to services in order to support the improvement of their health and wellness.
Qualifications:
● Master's degree in a qualifying health or human service field one year experience OR
● Bachelor's degree in a qualifying health or human service field 2 years’ experience OR
● Bachelor's degree in any field with 3 years’ experience as a Care Manager working with individuals with HIV/AIDS; Serious Mental Illness; Substance Use Disorders; Individuals transitioning from Institutional care OR
● A Credentialed Alcoholism and Substance Abuse Counselor (CASAC) and 2 years Care Management experience
● A minimum of 2 years’ health home care management experience preferred.
● Bilingual and able to read, write and speak Spanish proficiently preferred, but not required
Essential Functions:
● Engage referrals within 24 hours of receipt
● Maintain 12+ client caseload or as determined by DOH
● Ensure completion of the Eligibility Assessment from Health Home
● Work collaboratively with external providers and team members to continuity of care.
● Align with the vision of the agency while creating and maintaining an environment that encourages teamwork, collaboration, and positive work environment.
● Completes care plans for assigned clients and reviews and update goals and assessments accordingly.
● Participates in interdisciplinary meetings and treatment team meetings.
● Utilizes health information technology to document services and record progress in a timely and thorough manner.
● Assist clients in establishing and maintaining benefits and entitlements that foster health promotion.
● Ensures responsible delivery of client care per established Care Management policies and procedures.
● Participates in weekly health home team case conference meeting as well as bi-weekly one-on-one supervision with Supervisor.
● Maintains updated knowledge of other existing community resources and utilizes appropriately.
● Ensure every assigned client is contacted and billed for each month in accordance with DOH guidelines.
● Other duties as assigned by supervisor.

● Ability to analyze and interpret data and to handle problem resolution.
● Possession of a valid NYS driver’s license and a driver’s record considered acceptable by agency and insurance carrier.
● Continuous use of a reliable and insured vehicle.

Pay: $22.00 - $25.00 per hour
