Position Overview

Thrive HCR is seeking an experienced Master of Social Work (MSW) professional for an exciting 13-week travel assignment in Orange, California. This position focuses on acute care case management and discharge planning within a collaborative interdisciplinary team. You’ll support patients and families through complex hospital discharges, ensuring continuity of care and safe transitions.

Key Responsibilities

• Conduct initial psychosocial assessments within 24 hours of admission.
• Develop and coordinate individualized discharge plans for 20–25 patients.
• Collaborate with physicians, RN case managers, insurance teams, and post-acute care providers.
• Facilitate discharge to the most appropriate and cost-effective level of care.
• Educate patients and families about community resources, financial options, and available support programs.
• Maintain detailed and compliant documentation in Epic EMR.

Requirements

• Education: Master’s Degree in Social Work (MSW) required.
• Experience: Minimum 2 years of acute care case management or discharge planning experience.
• Certifications: MSW, BLS (preferred).
• Systems: Epic experience required.
• Travel Experience: Required.
• Float: Must be willing to float between like units as needed.
• First-Time Travelers: Not accepted.
• RTO: All time off must be disclosed during submission.
• Scrubs: Navy or Black.
• Vaccines: COVID-19 vaccine not required per facility guidelines.

Skills & Expertise

• Biopsychosocial assessments and care coordination.
• Crisis intervention, patient/family advocacy, and counseling.
• Medicare, Medicaid, HMO, and insurance coordination.
• Knowledge of protective services, hospice, and community resources.
• Collaboration with multidisciplinary healthcare teams for seamless discharge planning.
