Your Role as a Patient Care Coordinator...
As a key member of our Patient Care Team, you’ll be the friendly, empathetic voice patients look forward to hearing each month. You’ll ensure each patient receives the best experience possible through personal communication and attention to detail.
In this role, you will:
- Build and maintain relationships through monthly patient check-ins
- Coordinate with doctors’ offices for new prescriptions, refills, and prior authorizations
- Update and maintain accurate patient records (medications, insurance, contact info)
- Collaborate with the pharmacy and finance teams to resolve delivery or billing issues
- Help patients navigate insurance changes and pharmacy transitions
- Stay up to date with all required company training and compliance standards
What We’re Looking For
- Bilingual in English & Spanish (required — fluent speaking ability)
- 1+ year of customer service experience (healthcare or pharmacy preferred)
- Professional phone etiquette and excellent communication skills
- Strong data entry, organization, and multitasking skills
- NY State Pharmacy Technician license (preferred, not required)
- A caring, patient, and empathetic attitude with a genuine desire to help others
