Travel Required: The Intake Coordinator’s office is located in Medford, N.Y. but job responsibilities will require for the individual to travel throughout Nassau and Suffolk County. Reliable car a must.
Salary: $45,000 - $48,000
Position: Full-time; 35 hours per week; (does not include lunch), in-person.
Applicants will be trained.
Basic Function: The Housing Intake Coordinator is responsible for managing the intake process for nursing home residents seeking to qualify for OHS housing subsidy. This role involves assessing eligibility, coordinating with various departments, and ensuring a smooth transition for residents into appropriate housing. The Housing Intake Coordinator will also determine if an applicant meets OHS program eligibility criteria and will also conduct regular housing workshops/ outreach throughout the Long Island region.
Education and Qualification:
· Bachelor degree or Associate degree in Social Work, Human Services, or a related field.
· Minimum of 2 years of experience in housing services, social work, or a related field.
· Strong organizational and time management skills.
· Exceptional written communication and interpersonal skills.
· Ability to work independently and as part of a team.
· Knowledge of housing programs and state/county regulations.
· Compassionate and empathetic approach to working with nursing home residents and their families or informal supports.
· Must be computer literate of basic Microsoft Programs.
Key Responsibilities:
· Conduct initial intake assessments for nursing home residents applying for OHS housing subsidy.
· Review and verify documentation to determine eligibility for housing programs.
· Coordinate with nursing home staff, social workers, and external agencies to gather necessary information and documentation.
· Maintain accurate and up-to-date records of all intake activities and client interactions.
· Provide residents and their families with information about available housing options and program requirements.
· Assist residents in completing application forms and gathering required documentation.
· Schedule and conduct interviews with residents to assess their housing needs and preferences.
· Ensure compliance with all relevant regulations and guidelines.
· Provide regular updates and reports to the Housing Program Director(s) on intake activities and client status.
· Participate in training and professional development opportunities to stay current with housing policies and best practices.
· Participate in community outreaches, presentations, and workshops, as assigned.
· Attend agency’s in-service training, as well as any other agency related activities and off-site trainings.
· Perform other program-related duties as assigned.
· Attend mandatory NYAIL meetings/conferences.
