Job Title: MSW (Masters of Social Work)
Shift: Day 5x8-Hour (08:00 - 16:30)

Summary: The Master of Social Work (MSW) is responsible for providing professional social work services, including psychosocial assessments, care planning, crisis intervention, counseling, discharge planning, and resource coordination. The MSW works collaboratively with clients, families, and interdisciplinary teams to support mental health, recovery, and social well-being. This role requires knowledge of trauma-informed care, case management, and evidence-based interventions for individuals across the lifespan.
Responsibilities:
- Conduct comprehensive psychosocial assessments for individuals, families, or groups.
- Develop and implement individualized care or treatment plans based on client needs, goals, and available resources.
- Provide short-term counseling, crisis intervention, and support to clients facing medical, emotional, financial, or situational stressors.
- Assist with discharge planning and coordination of services such as housing, substance use treatment, financial aid, transportation, and home care.
- Advocate for clients and help navigate complex healthcare, social services, and legal systems.
Requirements:
- 2 - Year experience Required,
- Epic experience Required,
- Acute care case management/discharge planning hospital experience,
- BLS Certification,
- LMSW.
Job Types: Full-time, Contract, Temporary
Pay: $53.00 - $57.00 per hour
