???? Position Overview

We’re seeking a compassionate Licensed Master Social Worker (LMSW) to provide care management and coordination services for members at designated HIV primary care facilities.

You’ll make a meaningful impact by helping individuals living with HIV, transgender patients, and those experiencing homelessness access comprehensive healthcare and social services.

???? Key Responsibilities

• Deliver care management, transition support, and service coordination to improve patient outcomes
• Develop individualized Plans of Care with patients, caregivers, and providers
• Provide education on HIV management, chronic disease prevention, and wellness
• Collaborate with multidisciplinary teams and community agencies
• Monitor and analyze utilization data to identify member needs
• Mentor and support Health & Wellness Advisors
• Ensure compliance with regulatory and organizational standards

✅ Requirements

• Master’s Degree in Social Work from an accredited institution – Required
• Active New York State LMSW License – Required
• 2+ years of experience in HIV care, case management, or care coordination
• Experience with managed care – Preferred
• Bilingual (English/Spanish) – Preferred
