Nurse Care Manager – Managed Long-Term Care (MLTC)
Location: Queens, NY (Hybrid: 4 days remote, 1 day field after onsite training)
Sign-On Bonus: $20,000 ($5,000 payable every six months, less taxes)

Key Responsibilities:
- Conduct in-home assessments and develop personalized care plans for members.
- Collaborate with members, families, caregivers, primary care practitioners, and interdisciplinary teams to ensure consistent, high-quality care.
- Authorize covered services and coordinate care across different payers.
- Monitor members’ needs and health status to maintain the most independent living situation possible.
- Advocate for members while promoting quality and cost-effective outcomes.
Qualifications:
- Current NYS RN License and registration.
- Associate's degree in nursing required; Bachelor of Science in Nursing (BSN) preferred.
- Minimum of 3 years of RN experience.
- Clinical expertise in geriatrics preferred; experience in managed long-term care required.
- Bilingual in Chinese, Cantonese, or Mandarin required.
Compensation & Benefits:
- Annual salary: $105,000 – $115,000.
- Eligible for a $20,000 sign-on bonus ($5,000 payable every six months, less taxes).
