What You’ll Do
- Support individuals with developmental disabilities through person-centered planning.
Assist clients with daily living skills, goal-setting, and accessing community resources. - Conduct basic assessments to identify strengths, needs, and personal goals.
- Collaborate with families, service providers, and interdisciplinary teams.
- Advocate for clients to ensure their voices are heard and their rights are respected.
- Maintain accurate documentation and case notes.
- Participate in team meetings and training to strengthen your skills.
- High School Diploma or equivalent (training is provided—no prior case management experience required).
Compassion, patience, and cultural sensitivity when working with diverse communities. - Strong communication and teamwork skills.
- Reliable transportation for community visits.
- Familiarity with local resources is a plus.
- Experience supporting individuals with developmental disabilities is helpful but not required—we’ll train you!
- Bilingual applicants are highly encouraged to apply.
$18.50 per hour + mileage reimbursement
