Job Type: Contract - 13 Weeks

Job Duties:

• Conduct psychosocial assessments and intake interviews to identify client needs, strengths, and barriers.
• Develop individualized care or service plans in collaboration with clients, families, and interdisciplinary teams.
• Coordinate with healthcare providers, community organizations, and government agencies to ensure comprehensive support services.
• Provide crisis intervention and short-term counseling as needed.
• Advocate for client rights and access to necessary services (e.g., housing, financial assistance, medical care).

Job Requirements:

• Will work as discharge planner for med/surg tele, ICU, ED, PCU, joint/spine, obs short stay, etc
• State License
• Worked in an acute hospital setting within the last 2 years

Schedule:

• Monday - Friday
• 08:00 - 16:30
• Occasional weekends
• Guaranteed 40 hours/week

Job Type: Contract

Pay: $43.00 - $46.00 per hour

Experience:
