About the Role

We are seeking a detail-oriented and compassionate Home Health Intake Coordinator to join our team. The Intake Coordinator serves as the first point of contact for patients, physicians, hospitals, and referral sources (Veterans Affair). This role ensures a smooth admission process for new home health patients by verifying eligibility, coordinating with clinical staff, and managing all referral documentation efficiently.

Responsibilities

Receive and process new patient referrals from hospitals, physicians, and facilities.
Review referral documents for completeness and compliance with Medicare, Medi-Cal, and private insurance requirements.
Verify insurance eligibility and authorization prior to admission.
Coordinate with the RN Supervisor, LVN Case Manager, scheduling team, and clinicians to ensure timely start of care (SOC).
Maintain accurate and organized intake logs, communication notes, and EMR entries.
Communicate professionally with referral sources, patients, and families regarding admission status and requirements.
Assist in obtaining physician orders, clinical documentation, and signatures needed for admission.
Support the clinical and administrative teams with data entry, phone triage, and general office duties as needed.
Ensure patient confidentiality and compliance with HIPAA and agency policies.

Qualifications

High school diploma or equivalent (required); associate degree or higher preferred.
Experience in home health, hospice, or healthcare intake/referrals (minimum 1 year preferred).
Knowledge of Medicare, Medi-Cal, and private insurance processes.
Strong communication, multitasking, and computer skills.
Experience with EMR systems such as Data Soft Logic preferred.
Excellent organizational skills and attention to detail.
