Pay range: CA - $90-$100K annually, all other states - $80-$90K
Shifts: Tuesday - Saturday, 8a-5p PST
Sunday - Thursday, 8a-5p PST
Location: May reside in any of the following states, but must be willing to work west coast hours:

The Quality Care Manager is responsible for reviewing patient assessments to ensure appropriate and effective care is provided to each patient in compliance with agency policies, procedures, and regulatory requirements. Trends or patterns of inaccuracy identified by the QCM will be communicated to the Quality Director to assist in the development of the performance improvement and education plan.

Responsibilities:
Clinical Documentation & Compliance
- Review documentation and Plan of Care for accuracy, completeness, and alignment with care focus, and physician face-to-face requirements
- Ensure documentation supports skilled need, medical necessity, and appropriate visit utilization
- Confirm clinical documentation supports recertification and discharge decisions
- Review additional orders for appropriateness and locks HOPE assessments timely
- Reinforces agency processes when providing feedback on documentation deficiencies or HOPE corrections
- Maintains working knowledge of hospice regulations, CHAP standards, conditions of participation, and HOPE assessments
- Partners with the Director of Quality and Education on initiatives to improve HOPE accuracy and patient outcomes
Collaboration & Communication
- Communicates effectively with the healthcare team, leadership, and other departments
- Attends Case Conferences and department meetings as required
- Ensures adequate workflow coverage across offices, including on-call rotation for holidays/weekends
- Prioritizes and completes all workflow tasks daily to prevent delays in care and support timely billing and month-end close
- Demonstrates strong organizational skills, attention to detail, and problem-solving abilities
- Follows up diligently on outstanding tasks and adheres to agency practices
- Must be a graduate of an accredited School of Nursing
- A current unencumbered license to practice as a Registered Nurse in California is required. You will need to be licensed in OR, UT, ID and NV as well. Mission will support you in obtaining licensure in these state after you begin employment
- Minimum of 1 year of hospice experience with working knowledge of Medicare Hospice Conditions of Participation, HOPE Assessments, and Homecare Homebase
- Excellent written and oral communication skills required
- Ability to handle stressful situations/deadlines
- Strong computer and software skills with knowledge of Home Care Home Base highly preferred
- Self-starter, goal directed with ability to work independently and stay focused without on-site supervision
- Ability to communicate effectively with agency staff, upper management, and other members of the health care team
