POSITION SUMMARY

The Social Worker will provide support and counseling services for men and women who are coming home from incarceration in the Solano County Jails. These services include conducting one-on-one counseling and some group sessions on recovery and/or mental health. Responsibilities include conducting client intake assessments, evaluating client’s needs, creating treatment plans, documenting progress, and maintaining client notes. One social worker will be hired to work as part of a small staff.

ORGANIZATIONAL SUMMARY

Exodus Project launched in February 2019 as a program of the Society of St. Vincent de Paul Sacramento. The program has grown to a staff of 25 between Sacramento and Solano Counties, providing mentoring, housing, employment services, and other resources for men and women returning to the community from incarceration. The goal is to reduce recidivism, alleviate poverty, and reunify families through a holistic approach that emphasizes human dignity and self-awareness.

In Solano County, Exodus Project assists justice-involved clients through contracts with the County.

DUTIES AND RESPONSIBILITIES

· Meet with clients while in custody and conduct mental health evaluations (video visits)

· Provide short or moderate term one-on-one counseling services post-release (in office or by video)

· Plan, coordinate, manage, and implement treatment plans for clients

· Monitor and evaluate clients’ progress and modify treatment plans accordingly

· Conduct occasional groups with clients on topics like recovery, mental health, parenting, etc.

· Refer clients to community mental health services

· Maintain accurate, confidential, and up-to-date records and reports

· Perform other duties as assigned

QUALIFICATIONS

· Licensed clinical social worker, will consider ACSW close to licensure

· Experience working with the justice involved, homeless, mentally ill, and/or substance abuse population or other relevant social service work

· Experience preferred with client assessment, case management, and medication management

· Previous experience in a non-profit setting preferred

· Combination of education and experience will be considered

SKILLS AND ABILITIES

· Computer skills with proficiency in MS Office and database experience

· Strong verbal communications skills

· Strong organizational skills and time management

· Understanding of and compassion for the population we serve, and the issues related to incarceration, poverty, mental health, and addiction

· Capable of working independently and as part of a team to meet deadlines

· Sound judgment and discretion

· Proactive workstyle

· Comfort with faith-based/interfaith organization

HOURS AND COMPENSATION

1.0 FTE. $92,000-$102,00 annually based on experience and training.

Hybrid schedule possible.

Clinical supervision is provided.
