Working Hours:
Day Shift | 5x8 Hours | 08:00 AM – 04:30 PM
Weekend Coverage: Every Other Weekend
Holiday Coverage: Required (3 of 5 Major Holidays)

Position Summary:
The Medical Social Worker (MSW) will provide case management and psychosocial services to patients and their families. This includes conducting biopsychosocial assessments, discharge planning, care coordination, and connecting patients to community resources to ensure continuity of care.
Key Responsibilities:
- Perform comprehensive biopsychosocial assessments and develop care plans.
- Facilitate discharge planning and coordinate post-discharge services.
- Collaborate with multidisciplinary teams to ensure patient-centered care.
- Educate and advocate for patients and families regarding treatment options and available resources.
- Manage complex cases involving mental health, homelessness, substance abuse, or protective services.
- Participate in treatment team meetings and case reviews.
- Document all patient interactions in accordance with hospital standards.
Required Qualifications and Skills:
- Education: Master’s Degree in Social Work (MSW) – Required
- Experience: Minimum 2 years of acute hospital case management experience
- Certification: Current MSW License (CA)
- Systems: Epic charting proficiency

Preferred Qualifications:
- Experience in psychiatric hospitals or with involuntary holds (5150)

- Familiarity with admission criteria, benefits eligibility, and appeals/denials
