Daily Activities: Care Management Responsibilities
- Implement CalAIM and ECM: Drive person-centered care using CalAIM initiatives and Enhanced Care Management (ECM), adhering to all guidelines.
- Coordinate and Oversee Care Plans: Develop and implement Individual Care/Health Action Plans tailored to each client.
- Provide Flexible Support: Offer services where clients feel most comfortable—office, telehealth, or community settings.
- Engage and Connect: Proactively reach out to clients and connect them to the ECM program using various outreach methods.
- Assess and Address Needs: Conduct personalized needs assessments and share vital resources with clients.
- Organize and Coordinate Services: Seamlessly integrate services that align with ECM goals and CalAIM objectives.
- Manage Appointments and Transportation: Help schedule appointments and arrange transportation when needed.
- Support Legal Requirements: Aid clients with probation or parole requirements through progress reporting and documentation.
- Advocate for Clients: Work closely with healthcare professionals on behalf of clients, with their permission.
- Build Community Connections: Link clients with essential medical, social services, and support groups.
- Provide Client Support: Accompany clients to appointments when needed.
- Track Progress: Regularly evaluate client goals and adjust plans as necessary.
- Promote Mental Well-being: Foster mental health and well-being in all client interactions.
- Keep Records and Documentation: Maintain timely, accurate records in our EHR system, meeting compliance standards.
Team Responsibilities
- Collaborate with the Team: Actively participate in weekly staff meetings and supervision sessions.
- Learn Continuously: Engage in specialized training like Motivational Interviewing and ACEs Certification.
- Manage Data: Collect and input client data in EHR/EMR.
- Create Positivity: Maintain an uplifting, supportive atmosphere for both clients and team members.
Job Type: Full-time
Pay: From $26.00 per hour
