Concordia Private Care is seeking a Full-time Service Coordinator for our Valencia office. The Service Coordinator ensures that service is scheduled and delivered in an accurate, efficient, timely and consistent manner, maintaining quality service and positive interaction with patients, families, staff and referral sources.

QUALIFICATIONS for the Service Coordinator Position:

  • Interpersonal, communication and organizational skills. Evidence of leadership activities
  • Post-secondary education in business or the health field recommended.
  • Progressive two or three years of experience in health or related field
  • Outstanding communication skills to the extent that a positive and professional image is conveyed to staff, clients, families, referral sources and general callers.
  • Ability to maintain professionalism under deadlines
  • Particular orientation or experience working with older adults and their families
  • Ability to collaborate with health care providers and health related organizations to increase and improve services to the community

DUTIES AND RESPONSIBILITIES for the Service Coordinator position:

  • Supervises staff in all scheduling and disciplinary matters and reports all disciplinary issues to the Private Duty Manager
  • Maintains schedules for all staff, handling phone calls and messages to and from staff, patients, and referral sources
  • Arranges for “per visit” or temporary staff in consultation with Private Duty Manager.
  • Gathers and provides to staff appropriate and travel information required to provide care
  • Assists with receiving referrals and documents all relevant information
  • Identifies problems/concerns with patients and staff and reports them appropriately
  • Verifies schedules and reports for statistical and payroll purposes
  • Tracks recording activities such as admissions, visit reports, summaries, discharge plans of care and Concordia Visiting Nurse’s supervisory visit requirements and notifies staff appropriately
  • Completes clerical activities of patient/staff information
