Job Overview
We are seeking a dedicated and detail-oriented MDS Coordinator to join our team. The ideal candidate will play a crucial role in managing the Minimum Data Set (MDS) assessments and ensuring compliance with regulatory requirements. This position requires strong organisational skills, attention to detail, and the ability to work effectively in an office environment.

Responsibilities

• Conduct MDS assessments accurately and timely, ensuring compliance with all relevant regulations.
• Ability to identify significant changes timely and adhere to RAI process regarding significant changes.
• Ability to identify, implement and document toileting programs.
• Ability to educate and reinforce with the Interdisciplinary team Discharge Function scores.
• Demonstrate proactive interventions and assist in driving patient care.
• Maintain and update resident records in accordance with company policies and procedures.
• Demonstrate proficiency in chart reviews, identifying diagnoses and inputting ICD-10 codes per professional guidelines.
• Collaborate with nursing staff to gather necessary information for assessments.
• Development of care plans based on assessment findings.
• Manage data entry tasks, ensuring accuracy and confidentiality of resident information.
• Utilize Microsoft Office for documentation and communication purposes.
• Provide clerical support as needed, including typing reports and correspondence.
• Maintain an organized filing system for both electronic and paper records.
• Exhibit excellent phone etiquette when communicating with residents, families, and healthcare professionals.

Qualifications

• Previous MDS, PPS and careplan experience in LTC setting.
• Experience with IQIES.
• Experience conduction weekly Utilization Review meetings.
• Proficient in ICD-10 coding
• Excellent organisational skills with the ability to manage multiple tasks efficiently.
• Strong typing skills with attention to detail in documentation
• Demonstrated phone etiquette skills for effective communication with various stakeholders.
