Searching for a Care Manager/Care Coordinator:

The Care Manager/Care Coordinator is an integral part of the management team who oversees and ensures that:

• Clients & families are 100% satisfied with the quality of care received.
• Individual performances, for their team and staff, are properly monitored, reviewed and managed to ensure their responsibilities are met and all company procedures, protocols, policies are followed in an effective and professional manner.

Care Manager/Care Coordinator Requirements:

The ideal candidate will provide​ high-quality patient care in various settings, including a Home Care setting and an Independent Living​ Community.

• MUST live on Maui.
• Experience Needed: RN, LPN OR Certified Nurse Aide (CNA= certification + minimum 2 yr of In-Home Care or Long Term Care experience)
• Must be highly proficient and able to perform, know and teach all nurse aide skills.
• Must be available to work a flexible schedule, including weekdays, weekends, and all holidays as needed.
• Must have a valid driver's license, a reliable vehicle, and be willing to commute island-wide wherever client(s) reside.
• Must pass a thorough background check.

​Duties & Responsibilities Include:

• Daily Management of Assigned Client Caseload
• Timely Communication of Updates to aides, clients and client’s family
• Communication with the General Manager (Supervisor for managers)
• Direct/Hands on Care as Needed - Bonus Pay for emergency coverage outside of normal work schedule (5 days/week 8am-5:30pm)
• Assist in Preparation of Client’s Home In Case Of A Natural Threat
• Prepare Staff For Emergencies
• Daily Reading Client Charts / Documentation/Record Keeping
• Follow Up with Clients and Staff
• Complete Paperwork During Initial Meetings
• After Hours Phone Support
• Client Drop-Bys
• Plan of Care (POC) Development and Reassessment
• Staff Updates / Supervisor Log Notes
• Assure Quality of Client Care / Staff Performance Assessments
• Client & Employee Advocacy
• Facilitate and Oversee On-Boarding - New Cases
• Attend All Mandatory Training
• Maintain HIPAA Compliance

Care Manager/Coordinator Schedule:

• Sunday - Thursday or Tuesday - Saturday
• On call
• Overtime
• Weekend availability

** The Manager is required to sit or stand for long periods of time; frequently walk, kneel, lift and carry up to 50 pounds. The employee must also occasionally be able to push wheelchairs with heavier patients, talk and listen for extended periods of time. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.

Job Type: Full-time

Pay: $60,000.00 - $65,000.00 per year
