Belmont Manor is Family owned and operated skilled nursing facility. We have provided a warm, home-like setting for both long-term and short-term residents for over 50 years. We believe in fostering a positive culture that supports both professional growth and personal well-being for our staff.

As one of our Social Workers, you will plan and organize social services for our residents. You will ensure that the medically related emotional and social needs of each resident are met, helping them and their families navigate the transition to and through long-term care. The focus of your work will be on our short term residents.

Key Responsibilities:

  • Resident and Family Support: Provide counseling, emotional support, and advocacy for residents and their families. This includes assisting with the adjustment to placement and managing crisis situations.
  • Care Planning: Oversee and participate in interdisciplinary care plan meetings to address residents' social and emotional needs. Complete initial and comprehensive psychosocial assessments.
  • Discharge Planning: Coordinate all aspects of discharge planning to ensure a smooth and safe transition back to the community or another setting, including arranging for home care services, medical equipment, and follow-up appointments.
  • Regulatory Compliance: Ensure the social services department operates in accordance with all federal, state, and local regulations and standards.
  • Team Leadership: Supervise, train, and evaluate social services staff. Develop departmental policies and procedures.
  • Community Liaison: Serve as a liaison between residents, families, staff, and community resources. Maintain a comprehensive list of community services for resident referrals.
  • Grievance Resolution: Document and follow up on all resident and family grievances in a timely manner.

Qualifications and Requirements

  • Education: Bachelor's degree in Social Work or a related human services field. A Master’s degree in Social Work is preferred.
  • License: Must possess a valid Massachusetts Social Worker License
  • Experience: A minimum of 3 years of experience in a healthcare setting, preferably with experience in a Skilled Nursing Facility (SNF) or long-term care environment.
  • Knowledge: Familiarity with federal and state regulations, including the MDS (Minimum Data Set) and Medicare/Medicaid processes.
  • Skills:
  • Strong communication and interpersonal skills.
  • Excellent organizational and documentation abilities.
  • Effective leadership and problem-solving skills.
  • Deep empathy and compassion for the elderly.
