Responsibilities
Provide social support and resources to clients in need.
Conduct assessments to identify client needs and goals.
Develop and implement individualized care plans for clients.
Engage in continuous professional development and training.
Set and achieve personal performance targets and goals.
Collaborate with healthcare professionals to coordinate client care.
Monitor client progress and adjust care plans as necessary.
Advocate for clients' rights and access to services.
Maintain accurate and confidential client records.
Utilize data and metrics to track client outcomes and success.
Participate in team meetings to discuss client cases and strategies.
Foster relationships with community resources and organizations.
Experience/Qualifications
Master's degree in Social Work or related field required.
Commitment to continuous professional development and learning.
Experience in case management and client advocacy, preferably in social services.
Proven ability to develop and implement individualized care plans.
Ability to work independently while achieving personal performance targets.
Ability to advocate for clients' rights and access to necessary services.
Strong communication skills for effective client and team interactions.
Experience collaborating with healthcare professionals for coordinated client care.
Strong organizational skills to maintain accurate and confidential client records.
Familiarity with data tracking and metrics for client outcome evaluation.
