Responsibilities

• Provide social support and resources to clients in need.

• Conduct assessments to identify client needs and goals.

• Develop and implement individualized care plans for clients.

• Engage in continuous professional development and training.

• Set and achieve personal performance targets and goals.

• Collaborate with healthcare professionals to coordinate client care.

• Monitor client progress and adjust care plans as necessary.

• Advocate for clients' rights and access to services.

• Maintain accurate and confidential client records.

• Utilize data and metrics to track client outcomes and success.

• Participate in team meetings to discuss client cases and strategies.

• Foster relationships with community resources and organizations.

Experience/Qualifications

• Master's degree in Social Work or related field required.

• Commitment to continuous professional development and learning.

• Experience in case management and client advocacy, preferably in social services.

• Proven ability to develop and implement individualized care plans.

• Ability to work independently while achieving personal performance targets.

• Ability to advocate for clients' rights and access to necessary services.

• Strong communication skills for effective client and team interactions.

• Experience collaborating with healthcare professionals for coordinated client care.

• Strong organizational skills to maintain accurate and confidential client records.

• Familiarity with data tracking and metrics for client outcome evaluation.
