Overview
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will possess a strong commitment to improving the lives of individuals and families through effective counseling and support services. This role requires a deep understanding of mental health, behavioral health, and social work principles, as well as the ability to work collaboratively with clients from diverse backgrounds. The Social Worker will engage in various therapeutic practices, case management, and community outreach to provide comprehensive care.
Responsibilities
- Conduct assessments and evaluations to determine clients' needs and develop appropriate treatment plans.
- Provide individual and group counseling sessions utilizing techniques such as cognitive behavioral therapy, psychodynamic therapy, and motivational interviewing.
- Collaborate with multidisciplinary teams to coordinate care for clients, including those with developmental disabilities, PTSD, addiction issues, and other mental health concerns.
- Maintain accurate medical documentation in compliance with HIPAA regulations and ensure the confidentiality of client records.
- Engage in crisis intervention and management strategies to support clients in acute distress.
- Facilitate family counseling sessions to strengthen relationships and improve communication within families.
- Participate in clinical research initiatives to enhance service delivery and contribute to the field of social work.
- Utilize EMR systems for data collection, patient assessment, and tracking progress throughout treatment.
- Advocate for clients' needs within healthcare systems and community resources.
Skills
- Proficiency in psychotherapy techniques including individual/group counseling, crisis intervention, and behavioral therapy.
- Experience working with children and families in various settings such as pediatrics or special education.
- Knowledge of medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, and medical records management.
- Familiarity with home & community care practices as well as geriatric care principles.
- Strong skills in case management, diagnostic evaluation, patient care, and intake processes.
- Ability to conduct clinical research and data collection effectively.
- Experience working with individuals facing challenges such as addiction or developmental disabilities.
