We are a Federally Qualified Health Center (FQHC) that provides Article 16 and 28 clinic services at 8 locations across Suffolk County, New York. We are a multi-specialty healthcare provider caring for our community through Primary and Specialty Medical Care, Dental, Behavioral Health, Audiology, Physical Therapy, Occupational Therapy and Speech Language Pathology. Licensed Clinical Social Worker (LCSW)

Essential Duties & Responsibilities

  • Provides psychosocial assessments as referred.
  • Provides mental health intake assessments as referred.
  • Maintains close liaison with other care providers, especially psychiatrists involved in patient’s treatment.
  • Prepares Initial Assessments, Treatment Care Plans, Treatment Care Plan Updates, daily notes, discharge notes, etc., per Agency policy, in a timely manner.
  • Will provide relevant information about the PCMH program including brochures, that describe the values and goals of the program to patients. The information disseminated will be based upon the person’s role as it relates to patient care within LISH
  • Attends necessary in-service education programs.
  • Means and ability to travel between Agency buildings
  • Maintain licensing status in NYS by completing 36 hours of continuing education (CE) from a NY Department approved provider during each three-year registration period.

Education and/or Experience

Master’s Degree from an accredited School of Social Work in the United States

Minimum 2 year’s clinical experience

NYS Licensed Clinical Social Worker

Necessary Skills & Abilities

  • Language Skills
    • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.
  • Mathematical Skills
    • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
  • Reasoning Ability
    • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
  • Computer Skills
    • To perform this job successfully, an individual should have basic to intermediate knowledge of Microsoft Office software (Outlook, Excel, Word), as well as Practice Management and Electronic Medical Record software.
  • Other Skills and Abilities
    • Experience working in the field preferred. SCIP-R certification and CPR training a plus.
