Job Description
As a Community Liaison , you will be the bridge between Prudential Home Health and local hospitals, clinics, community centers, churches, Veterans centers, and patients/families. You will:

• Build and maintain strong community relationships
• Coordinate outreach programs, workshops, and events
• Serve as the primary point of contact for community inquiries and referrals
• Represent Prudential at local events and health fairs
• Collaborate with marketing and operations teams to maximize outreach impact

Requirements:

• 2+ years experience in community outreach, healthcare liaison, or patient advocacy
• Strong networking and communication skills
• Ability to manage multiple initiatives independently
• Active driver’s license and reliable transportation
• Availability during 8A–5P with occasional weekend events
• Proficient in Microsoft Office & Google Suite

Pay: $18.00 - $22.00 per hour
