Job Title: Social Worker / Case Manager
Location: New York, NY 10016
Type: Contract
Shift: 9:00 AM – 5:00 PM
Pay: $1,518 Weekly

Job Summary:

"Navitas Healthcare, LLC" is seeking Social Worker / Case Manager for an exciting job in New York, NY.

Responsibilities:

  • Provide social services to clients requiring medical, financial, or institutional support.

  • Assess client needs and develop individualized social service plans.

  • Determine eligibility for social service programs and make appropriate referrals.

  • Facilitate patient placements into nursing homes and coordinate ongoing case management.

  • Utilize Careport for referrals and document activities in EPIC (preferred).

  • Conduct counseling sessions and investigatory activities to support client well-being.

  • Collaborate with healthcare providers, social service agencies, and community resources.

Qualifications:

  • Bachelor’s Degree from an accredited college or university (required).

  • Minimum of 3 years of related experience.

  • Direct experience with nursing home placements and case management.

  • Experience with Careport and EPIC systems (preferred).
