RN Care Coordinator:
Location: Jamestown, ND
Shift: Varying hours
Job Schedule: Full Time
Weekly Hours: 40

JOB SUMMARY:
The purpose of the RN Care Coordinator is to coordinate and oversee the medical/nursing, social and developmental needs of children with intellectual and/or development disabilities and medical complex needs for the individual’s overall plan. He/she acts as an advocate for the individual and ensures completion of monitoring functions along with the other team members. This position assumes authority, responsibility, and accountability for the coordination of services and programming for the individuals is carried out in conjunction with other members of the team using all aspects of information gathered. The position reports to the Family Services Manager, however, with support from the Director of Nursing uses independent clinical judgment to provide clinical case management.

QUALIFICATIONS (minimum requirements)

Education and/or years’ experience required:

Graduate of an accredited school of nursing as a Registered Nurse, minimum of associate’s degree
Current and unencumbered RN license or eligible to achieve
One year experience in the care of medically complex children/individuals

Preferred Skills and Abilities:

Bachelor’s degree in nursing
Master’s degree in nursing or related business field
Two years’ experience in the care of medically complex children/individuals
Pediatric critical care experience preferred
Working knowledge of advanced technology and specialized equipment in the care of medically complex children/individuals

Supervision of others/decision making aptitude (if applicable): This is position is not responsible for supervision.
