About Job

Gurwin Healthcare System is a full continuum of care providing extraordinary care and caring to the people that we serve. Set on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis. Sharing the campus is Gurwin’s Assisted Living community (Gurwin Jewish ~ Fay J. Lindner Residences), our independent living community (Fountaingate Gardens) and our Gurwin Certified and Licensed Home Care Agencies.

Our Adult Day Care Program is seeking a Social Work Admissions and Marketing Coordinator.

EDUCATION AND EXPERIENCE REQUIREMENTS:

MSW from an accredited School of Social Work preferred

Licensed by the New York State Education Department preferred

At least one year experience in Long Term Care; Adult Day Health Care preferred

Bilingual preferred

PRINCIPLE FUNCTIONS:

Performs social work duties, which include but not limited to, helping the registrant and/or his caregiver with social, emotional, economic and rehabilitation problems.

Working with the Interdisciplinary Team in planning care and providing intervention through direct services.

Performs all other related duties as assigned. May be assigned to supervise undergraduate or graduate social work students or volunteers.

Identify and build effective network of community and professional sources for potential registrant referrals.

Marketing Hispanic Program to community resources.

DUTIES AND RESPONSIBILITIES:

Initiates the Admission Process; takes inquiries, assesses potential registrants, conferences with Director, Nursing Supervisor, and Social Work Supervisor to determine appropriateness for Adult Day Health Program, and initiates the chart where required.

Provides direct social work services and counseling to registrants, families/caregivers, (Individually or in a group setting as appropriate).

Participates in the biannual Interdisciplinary Care Plan Meeting or sooner if required.

Maintains community resource file and makes appropriate referrals for registrants or caregivers as requested.

Represents the Gurwin ADHP at professional or community meetings.

Various other duties as assigned.
