Social Worker / Business Development Manager

Serving Ponte Vedra, St. Johns County, Nocatee & Nearby Communities
Pay: $40,000 – $50,000 per year + commission

Premier Homecare Services is expanding and looking for a Business Development Manager to support our growth in Ponte Vedra, St. Johns County, Nocatee, and nearby areas.

What You’ll Do:

• Build strong, trust-based relationships with referral sources like hospitals, skilled nursing facilities (SNFs), rehabilitation centers, and physician offices across the region.
• Lead outreach efforts to grow our presence in the local healthcare community.
• Host community and professional education sessions about our home care services.
• Collaborate with marketing to drive awareness in Ponte Vedra, St. Johns County, and Nocatee.
• Maintain organized referral data and lead tracking via CRM tools.
• Report weekly on progress, opportunities, and performance metrics.
• Attend local events and represent Premier Homecare Services proudly.

What We’re Looking For:

• LMSW or LBSW preferred with experience in the senior care or healthcare industry.
• Candidates with backgrounds as an RN, LPN, PT, or OT are also encouraged to apply.
• 3–5 years of business development experience, ideally in home care or healthcare.
• Strong local network across healthcare providers and facilities.
• Great communication and presentation skills.
• CRM and PowerPoint proficiency.
• Experience working with elderly populations.
• A genuine passion for senior care.
• Bilingual is a plus, but not required.

• Base Salary: $40,000 – $50,000/year
• 3% commission on revenue for all referrals (for the lifetime of each client!)
