- Monitor and respond to Bamboo Patient Ping with appropriate next steps regarding patient inpatient admissions. Notification of admit status also via clinical documents in Athena as well as Epic.
- Performs TCM process including post d/c calls within appropriate time frame as well as scheduling HFU and ensure that visit is prepped accordingly with TCM outreach information.
- Provide CM outreach and appropriate workflows to patients enrolled in CM programs such as CCM/APCM
- Complete Tuck-In Calls to patients enrolled in Care Management programs
- Assist in VNA referrals and DME ordering with appropriate provider input
