Job Title: Prevention Care Manager

Location: Syracuse

Salary: $20.00 - $22.00

Classification: Full-time, Non-Exempt. (35 Hours) per week; occasional evenings and weekends are expected for workshops, conferences, and seminars.

Position Summary: The Prevention Care Manager provides low-barrier, trauma-informed care coordination for BIPOC and justice-involved people who use drugs. Using a harm reduction approach, the PCM supports participants in accessing essential services like healthcare, behavioral health, MOUD, support services, and other community-based resources, benefits, and entitlements. They work one-on-one with participants to assess their needs, develop action plans, and reduce barriers related to stigma, poverty, and systemic discrimination. This role plays a key part in improving health outcomes, reducing overdose risk, and helping participants stay connected to care through advocacy, trust-building, and consistent follow-up.

Minimum Qualifications:

  • Bachelor’s degree plus one-year of experience OR an equivalent combination of education and experience. Qualifying experience includes working in non-profit social services or healthcare setting providing direct client related services to diverse populations (e.g., people who use drugs (PWUD), justice involved individuals, those with mental health diagnoses, etc.). This is a critical component to the agency’s values and the work that is done in our community.
  • Case management and healthcare experience preferred.
  • Excellent organizational and time management skills.
  • Fluency in both Spanish and English is preferred.
  • Computer skills and proficiency in Microsoft Office are a must; Electronic Health Records systems experience is preferred.
  • Valid New York State driver’s license and access to a reliable vehicle with current insurance for work-related travel, training, outreach, and occasional client transportation. Employees are not required to own a personal vehicle but must have consistent access to one when necessary.
  • Sensitivity to HIV/AIDS, chronic illness, LGTBQ+ issues and a strong commitment to the mission, vision, and values of ACR Health and adherence to HIPAA compliance are essential.
  • Lived Experience of homelessness, substance use, as well as BIPOC and LGBTQI+ preferred.

Primary Responsibilities:

  • Incorporate best practices regarding confidentiality into all job duties and communications in accordance with Article 27-F and HIPAA, ACR Health policies and procedures and other applicable regulations. Protect agency data in accordance with confidentiality procedures and protocols. Observe and abide by the HIV Confidentiality Law and HIPAA.
  • Conduct comprehensive intake, risk assessments, and screenings for social determinants of health (SDOH) to inform individualized, client-centered action plans.
  • Develop individual care plans in conjunction with the client and related service providers.
  • Provide trauma-informed, low-barrier care coordination and advocacy for participants, addressing needs related to housing, food security, healthcare, behavioral health, MOUD, and legal system navigation
  • Collaborate with Engagement Specialists to conduct community-based outreach and identify, engage, and follow up with eligible individuals, including those recently released from incarceration or at high risk of overdose.
  • Facilitate warm handoffs and active referrals to culturally responsive service providers, including medical, mental health, and substance use treatment services.
  • Maintain contact to ensure sustaining motivation, timely acquisition of identified services and on-going assessment.
  • Maintain complete and accurate statistical information; prepare all required monthly and program reports as assigned; ensure thorough, up to date and complete client records.
  • Complete/submit all required documentation within designated time frames.
  • Authorize and arrange for necessary support, medical and stabilizing services for the client to develop and maintain effective health care.
  • Reduce barriers to services through persistent advocacy efforts and appropriate documentation of service gaps.
  • Complete all required staff, department, and ACR Health training, as well as a minimum of 24 hours annually of professional development training and certifications.
  • Coordinate and participate in case conferencing with community partners, justice system entities, and internal staff to support continuity of care.
  • Re-evaluate care plans on a regular basis to ensure continuity of services.
  • Ensure interventions are consistent with identified long/short term goals, care plans and needs assessment. Complete reassessments and modify care plan/goals as appropriate.
  • Provide crisis intervention when appropriate.
  • Maintain confidentiality of all clients, peers, and volunteers
  • Attend all required and recommended staff training and in-services.
  • Perform other duties as directed by the Health Homes Manager, Director of Care Management Services, Chief Program Officer and/or the Chief Executive Director
