Summary: The Social Worker is responsible for assessing and implementing the Plan of Care to address the psychosocial, emotional, coping, caregiving, and grieving needs of all Hospice patients and family and/or primary caregiver in collaboration with the Interdisciplinary Group.

Duties include:

  • Provide clinical social work services to patient/families admitted to hospice including but not limited to personal counseling, group (family) counseling, crisis intervention, and benefit determination, advocacy of patient/family needs, information and referral
  • Performs psychosocial assessment on each patient/family
  • Coordinate psychosocial needs and services with and for the patient/family/PCG and other resources available to the patient/family/PCG
  • Share in assessment, information visit, admission, service availability, treatment plan designs, implementation of services and evaluation responsibilities of the team
  • Assist with nursing home application and placement
  • Knowledge and understanding of state and federal regulations relating to hospice and palliative care
  • Knowledge of Medicare, Medicaid, and private insurance guidelines
  • Participate in interdisciplinary group meetings to update patient/family plan of care as needed
  • Participate in quality assurance program and perform chart audits as assigned
  • Works with and collaborates with all members of the interdisciplinary team as indicated
  • Maintain accurate, timely, and detailed documentation according to agency guidelines
  • Expectation of point of service documentation in patient care setting
  • Participate in programs of professional development related to hospice
  • Participates in staff development
  • Other duties as assigned by supervisor

Degree in social work obtained by successful completion of a prescribed course of study at a school of social work accredited by the Council of Social Work Education and the Education Department, and current certification or license by the Education Department to practice social work in the state of New York*. One year of social work experience in a health care setting, along with knowledge of Federal and State hospice/home care regulations preferred.

*Consideration may be extended to a candidate in the event the Master’s Degree or Bachelor’s Degree requirement is met but license is not yet obtained (or pending) and/or experience requirement is not. In this case, supervision by the social work manager, a licensed social worker, will also include ongoing review of care plans and clinical documentation until satisfactory completion of all minimum requirements (licensure, experience) are met. Individuals not licensed in New York State will be subject to Criminal History Records Check, as required by Department of Health requirements.
