The typical starting range for this position is between $63,448.32and $70,035.26 DOE.

Position Summary

The Choice Neighborhood Lead Case Manager (grant-funded) will organize, monitor, assist, and ensure that all delivered services by the Choice Neighborhood Initiative (CNI) case management team meet the needs of Marble Manor residents, as well as manage their own caseload that provides intensive case management to current and relocated Marble Manor residents.     Assesses family’s needs, develops an individualized plan, provides resources and referrals as needed, and tracks referrals and the outcomes for the family for eight (8) years, which is the duration of the Choice Neighborhood Initiative (CNI). This position requires a combination of administrative/office duties, basic training, monitor readiness, and field/outreach work. 

 

Duties and Responsibilities

  • Monitor and screen for daily eligible referrals, assign and monitor case managers' caseloads
  • Monitor, review, and assist Case Managers with master file review and assessment of data collected to assure all necessary documentation is collected for program services eligibility purposes 
  • Attend regularly scheduled agency-wide, community, and other meetings as assigned. Conduct all functions in accordance with Agency guidelines, policies, and procedures 
  • Arrange the work schedule or required hours to benefit the Department/Program with the immediate supervisor's approval.
  • Develops Individualized Care Plans (ICP), including health, education, and employment needs for each family member 
  • Provide assessment of the eligible resident to determine service needs 
  • Partners with community agencies to develop resources that assist program participants with quality of life concerns and to promote self-sufficiency 
  • Provide crisis interventions to help program participants avert crisis and to develop strategies to recognize potential triggers and create more productive outcomes 
  • Utilizes ICPs to help families prepare for self-sufficiency and plans relocation services
  • Assists program participants with finding community resources after relocation, such as employment opportunities and high-performing schools
  • Works with community partners to build safe
  • Demonstrate and promote a spirit of cooperation and teamwork through the organization
  • Continually look for ways to improve our processes and ability to serve our residents effectively 
  • Work closely with community agencies to help build and sustain partnerships. 
  • Lead team meetings to facilitate team building skills and other formats of staff development 
  • Flexibility to work outside normal work hours, as required

 

This position is eligible for participation in the collective bargaining unit (SEIU), representing certain Southern Nevada Regional Housing Authority employees.

 

Minimum Qualifications

Minimum qualifications help identify candidates most likely to perform successfully in the role. Applicants should demonstrate the following: 

 

Education, training, and work experience:

A bachelor’s degree in social work and business administration or a closely related field is preferred, or five (5) years of work experience in human services, case management, or social work, or an equivalent combination of education and experience. Public housing or similar related experience is preferred.

 

Licenses and Certifications

Valid Nevada State Driver’s License or ability to obtain one

 

Physical Ability:

This job requires seeing, talking, or hearing; continuous gripping or feeling with hands; typing/repetitive movement; frequent standing; frequent sitting; frequent reaching with hands and arms; occasional climbing or balancing; occasional walking; occasional stooping, kneeling, crouching, or crawling. Occasionally required to lift/and or move up to 20 pounds. The job requires depth perception.
