Position: Per Diem Hospice Medical Social Worker
Location: LOS ANGELES COUNTY
Languages: Bilingual is a plus!

AVH+ is a locally owned company with owners actively participating in the day-to-day operations of the agency performing as part of the team. We are fully staffed and looking to expand our team in order to support and anticipate our growing patient census's needs.

Role and Responsibilities:

• Provide emotional support, counseling, and resources to Spanish-speaking patients and families
• Assess the psychosocial needs of patients and develop appropriate care plans
• Collaborate with interdisciplinary team members to support holistic care
• Conduct home visits and maintain accurate records of patient progress

Qualifications:

• Master’s Degree in Social Work (MSW) from an accredited institution
• Licensed in the State of California (LCSW preferred)
• Bilingual in Spanish and English (required)
• Experience in hospice or palliative care (preferred)
• Compassionate, respectful, and committed to patient dignity
