Job Purpose:
The Foster Care Crisis Case Manager at Three Rivers Youth is responsible for providing immediate and ongoing support to foster children and their families during times of crisis. This role is crucial in ensuring the safety, stability, and well-being of children within the foster care system, while also collaborating with various stakeholders to develop and implement effective intervention strategies.
Key Responsibilities:
The Foster Care Crisis Case Manager at Three Rivers Youth is responsible for providing immediate and ongoing support to foster children and their families during times of crisis. This role is crucial in ensuring the safety, stability, and well-being of children within the foster care system, while also collaborating with various stakeholders to develop and implement effective intervention strategies.
Key Responsibilities:
- Respond promptly to crises involving foster children and families, providing immediate assessment and support.
- Develop and implement individualized crisis intervention plans to address the specific needs of each child and family.
- Coordinate with social workers, foster parents, and other professionals to ensure a cohesive approach to crisis management.
- Maintain accurate and detailed case records, documenting all interactions, assessments, and interventions.
- Provide ongoing support and follow-up to ensure the stability and well-being of children and families post-crisis.
- Advocate for the needs and rights of foster children within the system to ensure they receive appropriate care and resources.
- Participate in regular training and professional development opportunities to stay informed about best practices in crisis management and foster care.
- Collaborate with community resources and agencies to provide comprehensive support services to children and families.
- Communicate effectively with all stakeholders, including children, families, colleagues, and external partners, to facilitate positive outcomes.
Required Education:

Required Skills and Abilities:
- Master's degree in Social Work, Psychology, Counseling, or a related field.
Required Experience:
- Minimum of 2 years of experience in social services, preferably within foster care or child welfare systems.
- Experience in crisis intervention and case management.
- Proven track record of working effectively with children, families, and community resources.

- Excellent communication and interpersonal skills to interact with diverse populations.
