The duties that the incumbent of the vacancy will be expected to perform. Duties Description PRIMARY DUTIES AND RESPONSIBILITIES INCLUDE BUT NOT LIMITED TO: • Coordinate & Facilitate quarterly Comprehensive Functions Assessment (CFA) meetings and documenting minutes.• Participate in interdisciplinary care plan meetings and arrange hospital discharge meetings.• Review of interdisciplinary treatment team clinicians’ assessments to ensure consistency of Individuals’ information and care.• Conduct clinical tracking routinely.• Oversight of active treatment plans, schedules, and implementation of services & supports by staff to ensure that ICF regulations are followed accordingly.• Oversight of the Electronic Health Records (EHR) for recording the CFA plans and Individuals’ records. Maintain treatment records and documentation in accordance with program standards and requirements. • Advocate for Individuals’ rights and needs. Assist in obtaining their needs and required services.• Ongoing interactions with family members, advocates, and CAB representation for satisfaction of services, issues, and/or concerns.• Liaison with day programs, care providers, and other service providers to ensure coordination of care and treatment.• Facilitate referrals for new services.• Assist coordination with Financial Benefits & Entitlements Assistance & Management (FBEAM) and provide information to maintain Individuals’ Medicaid/Medicare and other entitled benefits.• Assist with end-of life planning and funeral/burial arrangements. • And other assignments by the TTL that are germane to the responsibilities.Coordinates the management of an assigned caseload with multidisciplinary staff for individuals who present with more complicated mental, social, emotional, behavioral, developmental, and addictive disorders, conditions, and disabilities; and may perform the full range of supervisory duties over Social Work Assistants 1 and direct care staff.LinkedIn: https://www.linkedin.com/company/nys-office-for-people-with-developmental-disabilities/posts/?feedView=all#LI-DA1

Minimum Qualifications Promotion: one year of service as a Social Work Assistant 1.Open Competitive: A bachelor's degree in a human services field and two years of social work experience.Substitution: Master's degree in a human services field can substitute for one year of specialized experience.

HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.• Please note that starting salary for this title reflects the Downstate adjustment. Posted Hiring Salary includes the sum of the statutory hiring rate for PS&T Grade 17 ($63,361) and Downstate Adjustment ($4,000).
