Client Care Coordinator
Key Responsibilities
- Client Needs & Care Plans:
Perform in-home consultations and needs assessments to create personalized, non-medical care plans tailored to individual client needs and goals.
- Scheduling:
Coordinate and schedule caregiver shifts, ensuring timely and appropriate staffing to meet client needs.
- Client Relations:
Act as the primary point of contact for clients and their families, addressing inquiries, concerns, and feedback in a timely and professional manner.
- Quality Assurance:
Conduct ongoing quality assurance visits to clients' homes and communicate with them to evaluate care plans, ensure client satisfaction, and maintain retention.
- Administrative Tasks:
Handle administrative duties such as record-keeping, processing client paperwork, and maintaining timely submission of care activity reports uploaded in clear care database.
- Problem-Solving:
Respond effectively to changes in client conditions or needs, making necessary adjustments to the care plan to prevent unnecessary services and maintain quality of care.
- Collaboration:
Work with internal supervisors and external providers to coordinate services and ensure a seamless care experience for the client.
Required Skills and Qualifications
- Strong organizational and communication skills.
- Empathy, compassion, and patience when working with clients.
- Ability to assess client needs and develop effective care plans.
- Skilled in coordinating and multitasking to manage various client and caregiver needs.
- Experience in home care, healthcare, or a related industry is often preferred.
