Job Summary:

The Case Manager (SUD) is responsible for providing comprehensive case management services to individuals struggling with substance use disorders. The case manager works closely with a multidisciplinary team to develop, implement, and monitor individualized treatment plans, facilitate group educational sessions, and provide ongoing support to clients and their families.

Responsibilities:

· Conduct comprehensive assessments to determine the needs and priorities of individuals with substance use disorders.

· Develop, implement, and monitor individualized treatment plans based on clients' needs and goals.

· Maintain accurate and up-to-date case records, documenting all services provided, changes in clients' conditions, changes to treatment plans, and interactions with clients in company’s EMR system (KIPU).

· Educate clients and their families about substance use disorders, treatment options, and community resources.

· Regularly review and update treatment plans to reflect clients' progress and changing needs.

Minimum Qualifications:

CDAC-l or CDAC- ll (CCAPP Certification), or equivalent.
2 years of case management experience in mental health, substance abuse, or related field.
Bachelor’s degree in psychology, sociology, or related field (preferred).

· Excellent interpersonal and communication skills, with the ability to build strong relationships with clients, families, and other service providers.

· Ability to handle high-stress situations and make critical decisions under pressure.

· Strong organizational skills, with the ability to manage multiple cases and prioritize tasks effectively.

· Proficiency in electronic health records systems and Microsoft Office Suite.

EMDR training and/or certification is a plus.
Valid California Driver’s License required.
Individuals in personal recovery must have a minimum of 2 years clean and sober.
Must pass a criminal background check as required by licensing authorities.
CPR, AED & First Aid Certification required.
Annual TB Testing required.

Regulatory & Compliance Expectations:

Maintain compliance with all state and federal regulations, including DHCS and Joint Commission standards.
Follow company policies on safety, ethical conduct, and confidentiality.
Participate in audits, training, and continuous quality improvement efforts.

Compensation & Benefits:

Compensation Range: $23 - $30 per hour (commensurate with experience and qualifications).
