About Us

Shanti Days is a cultural and wellness-focused adult day care center serving seniors 60+ in the Cleveland area. We provide transportation, Indian meals, wellness activities, and medical oversight, specializing in helping clients and families navigate Medicaid and insurance so they can attend at no cost.

Responsibilities

• Assist potential clients and families in applying for Medicaid Waiver / PASSPORT program for adult day services.
• Work directly with the Area Agency on Aging (AAA) case managers to process applications and ensure eligibility.
• Conduct intake assessments and develop individualized service plans.
• Serve as a liaison between families, healthcare providers, and Shanti Days staff.
• Maintain compliance with Ohio Department of Aging and Medicaid requirements.
• Provide culturally sensitive support to Indian and multicultural senior clients

Requirements

• Ohio LSW or LISW license required.
• Experience in gerontology, Medicaid waivers, or case management strongly preferred.
• Excellent communication and organizational skills.
• Ability to build strong referral relationships with hospitals, agencies, and community organizations.
• Bilingual (Gujarati, Hindi, or other Indian languages) is a major plus.
