DEFINITION:

The Licensed Clinical Social Worker (LCSW) provides short-term (10-12 sessions), goal-oriented mental health therapy to established adult BAYWELL HEALTH primary care patients presenting with mild-moderate mental health symptoms. The intention of the program is for the medical team and the behavioral health team to work together and actively collaborate to provide whole-person care to the people we serve.

QUALIFICATIONS:

1. Possession of a Master’s Degree in Social Work, Marriage and Family Therapy or Doctorate Degree in Clinical Psychology from a program that is accredited by the American Psychological Association.

2. Possession of a State of California license as a Licensed Clinical Social Worker or Psychologist OR California registration as an ASW or AMFT. (Supervision towards licensure provided by an LCSW.)

3. Knowledge of principles and practices of psychological assessment, personality theory, DSM 5, and evidence-based practice.

4. One year of experience providing psychotherapy to a predominantly BIPOC population.

5. Basic computer skills (i.e. familiarity with Microsoft Word, Excel, etc.) and ability to navigate the internet.

6. OCHIN Epic EMR experience.

DESIRED QUALIFICATIONS:

1. Familiarity in providing trauma-informed care.
2. Experience working in a health center setting.
3. Familiarity working in an electronic health record (bonus points for working with EPIC).
4. Spanish-speaking is preferred

EXAMPLE OF DUTIES:
1. Comply with established departmental policies and procedures, objectives, continuous quality improvement project guidelines.
2. Comply with all state and federal laws, and the professional code of ethics pertaining to the practice of psychology.
3. Conduct brief, non-stigmatizing screenings to evaluate the needs of adults and older adults for mental health and support services.
4. Perform comprehensive bio-psycho-social assessment for mental health, substance abuse, co-occurring disorders, intimate partner and family violence. Conduct follow-up as appropriate based on results of evaluation.
5. Provide behavioral health services utilizing brief treatment, problem-solving therapy, behavioral activation, and other Evidenced-Based Treatments, in a culturally, linguistically and age appropriate manner.
6. Provide ongoing consultation for primary care staff in appropriate mental health techniques for adults.
7. Provide linkage and referrals to community services as needed and work from an interdisciplinary approach and in collaboration with the PCP (Primary Care Provider), Consulting Psychiatrist, and Care Coordinator.
8. Participate in integrated and/or therapeutic support groups to address behavioral health and chronic health conditions such as diabetes and depression.
9. Participate with other staff in multidisciplinary staff meetings and other staff processes to achieve program development, staff development, program evaluation, program operations review, and other activities needed to maintain quality of early intervention services.
10. Maintain confidentiality regarding privileged administrative and patient information and complete required documentation in a timely manner.

GENERAL REQUIREMENTS:
1.Attends work regularly and punctually; is readily accessible during work hours to peers; limits absences from own duty stations and limits visits and activities which distract others from their performance of duties.
2.Works cooperatively with immediate supervisor, with department members, and with other Council staff persons, as required.
3.Treats all patients / clients / visitors with respect and courtesy.
4.Remains competent in skills required for position.
5.Will not provide services for non-BAYWELL HEALTH patients on any BAYWELL HEALTH sites.

6.During the initial thirty (30) days of employment, becomes and remains knowledgeable about and in compliance with the Clinic’s and Department policies and procedures in the following areas:Confidentiality of patient / client / fellow employee and supplier records.

8. Provides care appropriate to the age of the patients / clients / visitors served.

9. Is able to work and/or interact satisfactorily with a workforce and public which is ethnically and culturally diverse.

10. Is responsible for performing other assigned duties which are related to the above and which fall within the scope of this job description.
