About the Role

We’re seeking experienced Care Coordinators to support acute care case management and utilization review. You’ll collaborate with multidisciplinary teams, ensure patients get the care they need, and help keep the workflow smooth and efficient.

Roles & Responsibilities

• Coordinate patient care and communicate with multiple departments to ensure safe and effective care delivery.
• Perform utilization review and case management tasks.
• Assist patients, families, and staff with discharge planning, referrals, and follow-ups.
• Maintain accurate and timely documentation using Cerner EMR.
• Collaborate with physicians, nurses, and other healthcare team members to optimize patient outcomes.

Qualifications

• Minimum 2 years of experience in care coordination or case management in an acute care setting.
• Strong organizational, communication, and problem-solving skills.
• Ability to work independently and as part of a team.

Education & Licensure

• Active California RN license required.
• BLS certification required.

Knowledge & Skills

• Proficient with Cerner EMR and standard care coordination tools.
• Skilled in case management, utilization review, and patient advocacy.
• Strong interpersonal skills for collaboration with patients, families, and staff.
