330 Federal Qualified Health Center is looking for a Community Social Worker who promotes all management process in relation to social justice and community and patients rights of our Community Behavioral Health Clinic.

Responsibilities:

• Social Determinants of Health evaluation.
• Case discussion to different CCBHC Staff.
• Coordinates patient's hospitalization and other services.
• Search and coordinate community resources.
• Visit patients at home if necessary and/or requested by a member of the health team.
• Maintains communication with public and private agencies for the coordination of services and maintains a follow-up to ensure the provision of the required services.
• Realize bank of resources of help services.
• Develops a work plan that responds to the needs of the impacted community as its individuals and relatives.
• Develops treatment plans in conjunction with the interdisciplinary team for the solution of problems or deficiency of the individual or family.
• Mobilizes internees and external resources from the patient, individual or family to channel aid.

Requirements

• Bachelor's Degree in Social Worker
• Social Worker license is required.
• Experience in a related job.
• Excellent Communication Skills.
• Non-Profit Organization experience is a plus.
